Ultrasound in the Evaluation of Mammographic Abnormalities Related to Hormonal Replacement Therapy

Abstract
Purpose : to demonstrate the role of US in the evaluation of mammographic abnormalities or palpable mass related HRT. A. HRT related changes ; Benign : Effects of HRT which may be apparent on mammograms include symmetric or asymmetric increased in size of cysts. Common sonographic changes include symmetric or asymmetric increase of fibroglandular tissue, ductal dilatation with epithelial hyperplasia, and development or increase in size of fibroadenomas cysts, and carcinomas.
B. HRT related abnormalities ; Malignant : Although most studies have not shown and increased risk of breast cancer in these women, this treatment may result in mammographic changes that decrease the sensitivity of mammography in early detection of breast cancer and require additional evaluation to exclude malignant tumors. If mammographic abnormalities are detected, or breast related symptoms occur dense breast on mammogram during HRT, US can be helpful in differentiating fibroglandular proliferation from the true mass lesion, and in assessing the possibility of malignancy.
Thus US can decrease unnecessary cessation of HRT in case of benign asymmetric density on mammogram and also be helpful in the decision for follow-up versus biopsy.

Conclusion : For assessing the focal density and mass seen on mammograms or the palpable mass, US examination was of assistance in the decision for follow-up versus biopsy without cessation of HRT.
